Addressing the challenges that many of our members face on a daily basis is an important part of helping our communities achieve their best health. Factors such as where a person lives, their financial stability and education level, their access to food and more are known as “social determinants of health.” Priority Partners and Johns Hopkins Health Plans are committed to helping members and neighbors overcome these barriers through community action.

35th Annual Ocean City Community Health Fair
- Date: April 2
- Location: Roland E. Powell Convention Center
- What we did: Greeted members, provided free health information and resources, and assisted members who needed help renewing their Medicaid health benefits.
Crossroads Farmers Market
- Dates: April through November
- Location: Tacoma Park
- What we do: Hand out $5 in tokens for members to use on fresh, local food at the market.
Recent Community Grants
Gaithersburg Help
- Grant amount: $15,000
- Use: To buy and distribute nutritious foods in the Montgomery Village and Gaithersburg area, helping 150 families overcome food insecurity.
TasteWise Kids
- Grant amount: $11,000
- Use: To teach healthy eating habits to about 700 fourth graders at John Rodgers School and Patterson Park Public Charter School.
