What is a HSNI and why does it matter?

A HSNI is a short survey used to learn about your health. When you fill out the HSNI form, it helps Priority Partners to understand the best services to connect you with to improve your health. The more information we have about your health, the more personalized support we can offer you. This is why it’s important to fill out a HSNI form.

How does filling out a HSNI benefit me?

Maybe you have health goals you want to achieve, or maybe there are challenges keeping you from your best health. As a member, there are several types of support our Priority Partners care team can offer. We can arrange a care manager for you to work with to help improve a health condition. We may also be able to help you schedule appointments and provide rides to and from your doctor’s office.
