Prenatal Visit
Reward amount: $150
What is this visit? A checkup during the first trimester of your pregnancy. A doctor will check the health of you and your baby.
Who can earn this reward? Priority Partners members who are pregnant.** One reward per pregnancy in 2024.
How can I earn this reward? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have this visit during the first 12 weeks of your pregnancy. The visit must be between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by March 1, 2025 (offer code 24-23782).
Postpartum Visit
Reward amount: $150
What is this visit? A checkup to make sure you are healing well after having a baby.
Who can earn this reward? Priority Partners members who give birth.** One reward per pregnancy in 2024.
How can I earn this reward? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have this visit 7 to 84 days after delivery. The visit must be between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by March 1, 2025 (offer code 24-23783).
Lead Test
Reward amount: $75
What is this test? A blood draw to check how much lead is in your baby’s blood.
Who can earn this reward? Priority Partners members who are 1 year old.** One reward per member in 2024.
How can I earn this reward? You must be eligible for this service. Call your doctor to find out if you are eligible. Children must have this visit during the year they turn one year old. The visit must be between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by March 1, 2025 (offer code 24-23784).
SSI Yearly Doctor Visit (Adult)
Reward amount:
$100 for visits Jan. 1, 2024 to June 30, 2024
$75 for visits July 1, 2024 to Sept. 30, 2024
$50 for visits Oct. 1, 2024 to Dec. 31, 2024
What is this visit? A checkup to learn about your overall health or a sick visit.
Who can earn this reward? Priority Partners adult members who are ages 21-64 and on the SSI program (supplemental security income).** One reward per member in 2024.
How can I earn this reward? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have a checkup or a sick visit with your doctor. The visit must be between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by March 1, 2025.

Diabetes Eye Exam
Reward amount: $75
What is this exam? A dilated eye exam that checks the health of your eyes and vision.
Who can earn this reward? Priority Partners members age 18-64 who have diabetes.** One reward per member in 2024.
How can I earn this reward? You must have a diabetes eye exam between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by March 1, 2025 (offer code 24-23785).
Diabetic Kidney Evaluation (KED)
Reward amount: $75
What is this evaluation? A urine test to check for a protein called albumin.
Who can earn this reward? Priority Partners members age 18-64 who have diabetes.** One reward per member in 2024.
How can I earn this reward? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have a diabetic Kidney evaultaion between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by March 1, 2025 (offer code 24-23786).
Claim Your Reward
After your visit or test, go to prioritypartnersrewards.com to request your prepaid Mastercard. You will need this information:
- Offer code
- Date of doctor visit
- Your Priority Partners member ID number
- Doctor name
Once your submission has been verified, you will receive your prepaid Mastercard. It may take 6-8 weeks or longer to receive your reward. Submissions must be completed by March 1, 2025.
