Diabetes Eye Exam
Benefits card amount: $75
What is this exam? A dilated eye exam that checks the health of your eyes and vision.
Who can earn this? Priority Partners members age 18-64 who have diabetes.** One benefit card per member in 2025.
How can I earn this? You must have a diabetes eye exam between Jan. 1, 2025 and Dec. 31, 2025.

Diabetic Kidney Evaluation (KED)
Benefits card amount:
$75 for visits from Jan. 1 through April 30
What is this evaluation? A urine test to check for a protein called albumin.
Who can earn this? Priority Partners members age 18-64 who have diabetes.** One benefit card per member in 2025.
How can I earn this? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have a diabetic Kidney evaluation between Jan. 1, 2025 and April 30, 2025.

Lead Test
Benefits card amount: $75
What is this test? A blood draw to check how much lead is in your baby’s blood.
Who can earn this? Priority Partners members who have a lead test between their first and second birthday.** One reward per member in 2025.
How can I earn this? You must be eligible for this service. Call your doctor to find out if you are eligible. Children must have this visit between their first and second birthday. The visit must be between Jan. 1, 2025 and Dec. 31, 2025.

Postpartum Visit
Benefits card amount:
$150 for visits from Jan. 1 through April 30
What is this visit? A checkup to make sure you are healing well after having a baby.
Who can earn this? Priority Partners members who give birth.** One reward per pregnancy in 2025.
How can I earn this? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have this visit 7 to 84 days after delivery. The visit must be between Jan. 1, 2025 and April 30, 2025.

Prenatal Visit
Benefits card amount:
$150 for visits from Jan. 1 through April 30
What is this visit? A checkup during the first trimester of your pregnancy. A doctor will check the health of you and your baby.
Who can earn this? Priority Partners members who are pregnant.** One reward per pregnancy in 2025.
How can I earn this? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have this visit during the first 12 weeks of your pregnancy. The visit must be between Jan. 1, 2025 and April 30, 2025.

SSI Yearly Doctor Visit (Adult)

$100 for visits Jan. 1, 2025 to June 30, 2025
$75 for visits July 1, 2025 to Sept. 30, 2025
$50 for visits Oct. 1, 2025 to Dec. 31, 2025
What is this visit? A checkup to learn about your overall health or a sick visit.
Who can earn this? Priority Partners adult members who are ages 21-64 and on the SSI program (supplemental security income).** One benefit card per member in 2025.
How can I earn this? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have a checkup or a sick visit with your doctor. The visit must be between Jan. 1, 2025 and Dec. 31, 2025.

SSI Yearly Doctor Visit (Child)
Benefits card amount:
$100 for visits Jan. 1, 2025 to June 30, 2025
$75 for visits July 1, 2025 to Sept. 30, 2025

Who can earn this? Priority Partners child members who are ages 0-20 and on the SSI program (supplemental security income).** One benefit card per member in 2025.
How can I earn this? You must be eligible for this service. Call your doctor to find out if you are eligible. You must have a checkup or a sick visit with your doctor. The visit must be between Jan. 1, 2025 and Dec. 31, 2025.
